Position Title

Client Care Advisor / Client Care Coordinator – In-Home Care

Location

Goodyear

Main Responsibilities

  • Coordinate and oversee client in-home care services
  • Develop and update individualized care plans
  • Communicate with clients, families, caregivers, and healthcare providers
  • Monitor client satisfaction and quality of care
  • Conduct follow-up calls and home visits when needed
  • Ensure documentation and care records are accurate and compliant
  • Support caregiver training and performance improvement
  • Help keep clients safely living at home
  • Resolve client concerns compassionately and professionally
  • Assist with scheduling and service coordination

Qualifications

  • CNA certification or caregiving experience often preferred
  • 2–3 years experience in home care, healthcare, hospice, or senior care
  • Strong communication and organizational skills
  • Experience with EMR/home-care software systems preferred
  • Ability to work independently and as part of a team
  • Compassionate, patient, and detail-oriented personality
  • Reliable transportation may be required
  • Bilingual English/Spanish is sometimes preferred

Schedule & Pay

  • Full-time, Monday–Friday 830-5p occasional weekend or evenings 
  • Occasional on-call duties
